Pearl Street Center, soon to be Pearl Youth Residence, is moving and growing! This summer we will be relocating to a beautifully remodeled building and we will be expanding from 14 beds to 27!  


Pearl is one of 5 Children's Long-term Inpatient Programs (CLIP) in the state and we serve youth ages 12-18 who have demonstrated an inability to maintain safety in their home or community.  


Youth come to our secure program and engage in treatment to build their skills to be able to transition successfully home or to another less restrictive placement.  


The fitting nurse will join a close-knit multidisciplinary team consisting of psychiatrist, nurses, residential counselors, recreation therapists, case managers, certified peers, and individual/family therapists.  We also have the pleasure of working with amazing teachers from Tacoma Public Schools, equine therapists from Healing Hearts Ranch, and more.

ScheduleWe are currently filling positions for all shifts.  We may be able to accommodate 12-hour, 10-hour, or weekend double shifts.  Days and hours to be determined prior to interview.

Function: Provide nursing and medical services to the residents of the Children’s Residential Treatment Facility. Serves as a member of an organization that welcomes, engages, and serves individuals with complex wellness needs.


Responsibilities:



  1. Provide nursing and medical services to residents.

  2. Provide education regarding prescribed medication and medical treatment to residents, families, and staff.

  3. Document in the progress notes the provision of care and the clinical and behavioral observations of residents, using the electronic health record

  4. Administer and document medications adhering to state and program guidelines.

  5. Provide emergency medical care as needed.

  6. Receive and transcribe medical and medication orders and add medications to the electronic prescribing program

  7. Serve as a resource person for education and guidance on medications and medical treatments

  8. Provide medical information.

  9. Write medical summaries as assigned, using the electronic health record

  10. Implement nursing treatment approaches as designated through a nursing care plan.

  11. Observe and assess residents in terms of therapeutic response to medication and the nursing care plan.

  12. Assist residents in daily living activities.

  13. Coordinate medical activities on shift with assigned supervisor and team members.

  14. Assist in monitoring and documenting medical supplies, floor stock, and pharmacy communication.

  15. Document, monitor, and assist in all aspects of the laboratory process.

  16. Serve as a member of the Pearl Street Health Team.

  17. Assist in infection control activities and be a member of the infection control committee.

  18. Ensure the care, safety, and supervision of residents on the therapeutic milieu through positive interactions and direct proactive supervision.

  19. Utilize individualized and tailored care, in consideration of the specific needs and strengths of residents.

  20. Instruct, assist, and provide skill development to residents regarding daily living activities, life skills, social skills, vocational skills, and leisure/recreation skills and activities.

  21. Plan and lead educational and information groups for residents as assigned.

  22. Participate as a member of the multi-disciplinary team to design, implement, maintain, and evaluate individually tailored care plans for each resident.

  23. Interact professionally, cooperatively, and promptly with other team members in crisis situations.

  24. Establish and maintain a therapeutic relationship with residents and actively support the program by participating in activities during scheduled shifts.

  25. Work to ensure effective communication among team members during and between shifts.

  26. Coordinate with school and staff regarding outside medical appointments and activities.

  27. Transport and attend medical appointments with residents

  28. Work with team members to create and contribute to a positive learning environment for all.

  29. Document and submit reports on all accidents, special treatment approaches, and extraordinary occurrences.

  30. Report unsafe conditions and needed repairs to shift supervisor, program manager, or safety officer.

  31. Actively seek, schedule, and participate in clinical supervision.

  32. Actively participate at in-service training and supervision activities.

  33. Utilize appropriate problem resolution activities and supervisory support when necessary to resolve conflicts.

  34. Ensure that ethical and professional standards of practice are maintained.

  35. In addition to the above, perform other duties as assigned.


RN Duties:


In addition to the above duties RN’s are responsible for:



  1. Complete a comprehensive head to toe nursing assessment upon a resident admission

  2. Complete a suicide assessment upon admission and during any safety concerns

  3. Be available on-call when assigned to come in and complete an assessment of a resident involved in a seclusion or restraint situation.

  4. Respond in a timely manner when on call

  5. Complete nursing care plans for medical needs when needed

  6. Participate in team meetings around resident needs when asked

Qualifications





Competencies:



  • Demonstrates ability to assess and document urgent and ongoing medical issues associated with residents.

  • Ability to utilize general medical equipment to assess medical needs

  • Demonstrates the ability to give Intramuscular, Subcutaneous and Intradermal injections according to facility procedures

  • Demonstrates ability to prepare and administer oral, topical, sprays, medications using the five rights of medication delivery

  • Ensures laboratory tests are ordered, prepared, documented and sent to the Lab using appropriate agency protocols.

  • Communicates client needs to the Psychiatrist in a timely manner.

  • Schedules, transports and attends all outside medical appointments and communicates the results to the appropriate staff.

  • Updates and answers questions of family members about resident’s medical concerns in a timely manner

  • Demonstrates the ability to complete nursing assessments, document risk and notify.

  • Demonstrate effective communication both orally and in writing.

  • Ensure documentation and assessments are completed in a time manner, and documented appropriately

  • Demonstrates appropriate boundaries and values.

  • Demonstrates professional demeanor, behavior, and communication.

  • Promote whole health care as a key to people's recovery and quality of life.

  • Demonstrate empathy, hopefulness, and resourcefulness in communication with others

  • Use "recovery" language (ex. respectful, hopeful, person-centered, non-judgmental).


Qualifications:



  • BSN, RN. WA State Registered Nurse. One-year work experience with children and/or adolescents. Experience or knowledge of psychotropic medications. Psychiatric experience, child development, milieu treatment, crisis intervention, and individual, group, and family counseling experience.

  • Current CPR certification.

  • Willingness to obtain MHP designation upon hire



  • Skilled in MS Word and Outlook.

  • Strong analytical, interpersonal, written, and verbal communications skills.

  • Demonstrated ability to multi-task, prioritize deadlines, work independently, take initiative, and maintain confidentiality.

  • Valid Washington State driver’s license; proof of insurance.

Medical Equipment used: Blood Pressure Cuff, stethoscope, scale, oximeter, thermometer, otoscope,


Physical Requirements: Safe Crisis Management ability. Ability to participate in physical holds. Physical ability to be stationary and traverse in an office environment. Frequent walking and standing. Occasional moving up to 25 lbs. Ability to transport clients as required.


Working/ Environmental Conditions: Adolescent consumer facility. Maybe residential facility or inpatient milieu. Computer use. Indoor environment.
