Stony Brook Children's Hospital is seeking a full time Pediatric or Family Advanced Practice Nurse Practitioner to join our team in a full-time role. The position will entail time spent both with our Pediatric Palliative Care Team and our Coordinated Fetal Care Program to work to provide inpatient and outpatient pediatric palliative care consultation and prenatal visits. Our growing Pediatric Palliative Care team consists of two part-time physicians and a pediatric social worker, although we work closely with a larger institutional interdisciplinary team which includes child-life specialists, chaplaincy, psychology and social work.
Job Responsibilities:
Pediatric Palliative Care Team:
• Assist in communication and facilitated goals of care conversations for seriously ill pediatric patients.
• Facilitate implementing limitations of care when in accordance with family wishes, including documentation of MOLST per NY State regulations.
• Provide recommendations about symptom management and assist with medication titrations.
• Participate in daily team rounds.
• Communicate with care teams to facilitate high quality and compassionate care.
• Thoroughly document visits with adherence to billing practices.
• Conduct formal and informal education about palliative care.
• Serve as link integrating relevant research and best practices.
Coordinated Fetal Care Program (CFCP):
• Assist with coordination of care for pregnant women who have a fetal diagnosis requiring multi-specialty evaluation and management.
• Serve as a point of contact for pregnant women to provide support, reassurance and guidance.
• To serve as a liaison between all relevant clinical parties to optimize communication and patient care.
• To assist in maintaining RedCap Database of patients enrolled in CFCP.
• To assist with scheduling of consultations relevant to the fetal diagnosis both within and outside of Stony Brook Children's.
Education, Skills and Experience:
• Master's degree from an accredited program. Current certification as a Nurse Practitioner (NP) through their national certification bodies. Licensure as a Registered Nurse and Nurse Practitioner (NP) in New York State.
• At least 2 years of experience with at least 1 year of experience in pediatrics, hospice, oncology, palliative care, and/or ICU preferred.
• Commitment to palliative care philosophy.
• Outstanding communication skills with staff, the medical community, and patients and families.
• Comfort working with socio-culturally diverse patients/families.
• Ability to take initiative, work independently, multi-task and manage time well.
• Comfortable documenting in an electronic medical record (EMR).
• A professional and caring demeanor.
• Certification or willingness to certify in hospice and palliative care preferred.
Qualifications
Required Qualifications: Master's Degree from an accredited program. Current certification as a Pediatric Nurse Practitioner or Family Nurse Practitioner, Licensure as a Registered Nurse and Nurse Practitioner in New York State. At least two years of clinical experience.
Preferred Qualifications: At least 1 year of experience in pediatrics, hospice, oncology, palliative care, and/or ICU. Certification or willingness to certify in hospice and palliative care.
